Report on serious aviation incident at Hønefoss, Norway 10 June 2007 with Reims Aviation SA F182Q, LN-HOA

The aircraft was newly modified with a diesel engine and a new propeller when it after 11:20 flight hours lost its propeller during flight. An emergency landing was performed at a field. Nobody was hurt. The modification was approved by EASA. The investigation has among other findings, showed that there are possibilities for safety improvements regarding torque of bolts during installation, quality of mounting bolts and the approval process. AIBN is of the opinion that there should be a general requirement to document the torque (and other measured values) and AIBN has previously issued a safety recommendation related to this. The bolts used for this installation had a quality below what's expected for a critical part in the aviation industry. In this case, the modification increased the possibility for resonance while the tolerance for resonance was decreased. AIBN think that the interaction between engine and propeller is of major importance when certifying modifications like this.
